## Insurance

Most Hawai‘i insurance plans accepted. In network: HMSA PPO, HMSA Medicare Advantage,
Medicare, Kaiser Added Choice, Devoted Health, HMAA, UHA, MDX. Ohana Medicare,
AlohaCare, QUEST (Medicaid), VA, and Tricare Prime require a referral or prior
authorization from the patient's plan before the visit. Self-pay and CareCredit
financing are accepted.

Injury claims (billed to a third party, not health insurance — see
https://pacificboneandjoint.com/insurance/#injury-claims):
- Workers' compensation: accepted at all four clinics. Needed before the visit: date of
  injury, employer name and contact, confirmation the injury was reported to the
  employer, and the employer's WC carrier, claim number, and adjuster contact if assigned.
- Auto accident: Hawai‘i is a no-fault state; the patient's own auto policy PIP benefits
  usually pay first. Needed: accident date and police report number, auto carrier, policy
  and PIP claim number, adjuster contact, health insurance card (secondary), attorney
  contact if any.
- Personal injury (third-party liability): requires a letter of protection (LOP) or lien
  from the patient's attorney before scheduling. Documents can be faxed to (808) 439-6202
  or emailed to contact@pacificboneandjoint.com.

## Important honesty notes

- Not everyone is a candidate for surgery; evaluation required.
- PRP/BMAC (regenerative medicine) may relieve symptoms but are not a cure,
  and are usually not covered by insurance.
- Robotic technology assists the surgeon; it does not perform surgery on its own.
