Medical Coder / Biller

Pacific Bone & Joint is bringing billing administration fully in-house, and we are hiring an experienced Medical Coder / Biller to own it. You will code and submit claims for a busy orthopedic surgery and physical therapy practice, work our aged accounts to recover funds that are owed to us, and give the practice clear, regular billing reports it can act on.
This is a hands-on role with real ownership. You will sit with the team that sees the patients, work directly with our practice manager and physicians, and see the results of your work in the numbers every month.
Please note: This is a local, in-office position for a Hawai‘i resident who will work on-site with our team. We are not seeking outsourced billing companies, remote contractors, or agencies for this role.
What you’ll do
- ✓Code office visits, procedures, surgeries, injections, imaging, and physical therapy from provider documentation using current CPT, ICD-10-CM, and HCPCS codes, with correct modifiers.
- ✓Prepare, scrub, and submit clean claims to HMSA, Medicare, Medicare Advantage, QUEST plans, commercial payers, workers’ compensation carriers, and auto (PIP) carriers.
- ✓Post payments and adjustments, reconcile remittances, and keep patient balances accurate.
- ✓Work denials, rejections, and underpayments through to resolution — corrected claims, appeals, and payer follow-up.
- ✓Recover old funds: systematically work aged accounts receivable, identify unbilled or under-billed encounters, and bring older claims to closure before timely-filing limits.
- ✓Produce recurring billing reports for the practice manager and physicians — A/R aging, collections, denial trends, payer mix, and productivity — and flag problems early.
- ✓Answer patient billing questions with patience and clarity, and set up payment arrangements when needed.
- ✓Help keep documentation, coding, and billing practices compliant with payer rules and HIPAA.
What we’re looking for
- ✓Lives in Hawai‘i and can work on-site at our Honolulu clinic. Local candidates only — this role is not open to remote contractors or outsourced billing services.
- ✓Two or more years of hands-on medical billing and coding experience in a physician practice; orthopedic, surgical, or physical therapy billing is a strong plus.
- ✓Coding credential (CPC, CCS-P, CPB, or equivalent) preferred, or the experience to earn one.
- ✓Experience with Hawai‘i payers — HMSA, HMSA Medicare Advantage, Medicare, QUEST, UHA, HMAA — and with workers’ compensation and auto (PIP) claims.
- ✓ModMed (Modernizing Medicine) EMR and practice-management familiarity is a plus.
- ✓Proven track record working aged A/R and denials, not just submitting new claims.
- ✓Comfortable building and explaining reports in Excel or Google Sheets.
- ✓Organized, detail-oriented, and discreet with patient and financial information.
