Orthopedic Care for East Honolulu
Orthopedic Care for Patients in East Honolulu, Hawai‘i
East Honolulu runs from Kaimukī and Kāhala past ‘Āina Haina and Niu Valley to Hawai‘i Kai. Many residents stay active well into retirement — on the Koko Head stairs, on tennis and pickleball courts, on golf courses, and in the water. Many of the orthopedic problems we see from this area come from wear and tear: knees and hips that hurt on stairs, stiff shoulders, and arthritic thumbs. Sports injuries are common too.
The closest Pacific Bone & Joint clinic is in the Ala Moana Medical Building in Honolulu. It offers orthopedic evaluation, in-office X-ray and ultrasound, physical therapy, injections, and surgical planning. You can start with the least invasive reasonable option and move to more advanced treatment only if you need it.

Who will I see?
Pacific Bone & Joint is a team-based orthopedic practice. Depending on the reason for your visit, your care may involve an orthopedic surgeon, a physician assistant, a physical therapist, imaging staff, or another member of the orthopedic team. Surgical decisions and operations are handled by the surgeon. Other parts of your evaluation, rehabilitation, follow-up, injections, and recovery may involve other clinicians.
Common Conditions We Treat
What brings East Honolulu patients to us
Knee and hip arthritis
The most common reason East Honolulu patients come in. Care ranges from physical therapy, bracing, and injections to partial or total joint replacement.
Pickleball and tennis injuries
Calf strains, Achilles tendon problems, tennis elbow, and wrist injuries from falls on the court.
Rotator cuff and shoulder pain
From swimming, paddling, golf, and overhead work. Most cases start with rehabilitation.
Thumb and hand arthritis
Base-of-thumb arthritis, trigger finger, and carpal tunnel syndrome that make gripping and typing hard.
Fractures from a fall
Wrist and hip-area fractures from a simple fall can signal osteoporosis, so we look at bone health along with the break.
Meniscus and cartilage problems
Catching, locking, or swelling in an active knee, sometimes treatable with meniscus repair or cartilage restoration.
Physical Therapy and Rehabilitation
Orthopedics and physical therapy, coordinated
Many orthopedic conditions improve without surgery. PB&J Physical Therapy works inside our Honolulu and Waipahu/Kunia clinics, so you can move from diagnosis to rehabilitation with one coordinated plan, and your therapist and orthopedic provider can talk about your progress.
You do not need to be a surgical patient to do therapy with us, and Hawai‘i allows direct access to physical therapy without a physician referral (some plans still require one for coverage). Not every patient needs therapy; it is recommended when it fits the problem.

Common Questions
Questions from East Honolulu patients
Can I start with physical therapy instead of seeing a surgeon?+
Yes. Hawai‘i allows direct access to physical therapy, so you can begin with PB&J Physical Therapy at our Ala Moana clinic without a physician referral (some insurance plans still require one for coverage). If therapy is not enough, an orthopedic evaluation is available in the same practice.
Do you treat pickleball injuries?+
Yes. Calf and Achilles strains, tennis elbow, knee pain, and wrist fractures from falls are common among pickleball players. Treatment usually starts with rehabilitation and a gradual return-to-play plan, and imaging is used when the diagnosis is unclear.
I have knee arthritis. Am I headed for a knee replacement?+
Not necessarily. Many people with knee arthritis do well for years with strengthening, weight management, bracing, activity changes, and injections. When those stop working, partial knee replacement may be an option for arthritis limited to one part of the knee, and total knee replacement for more widespread damage.
Do you offer PRP?+
Yes, for selected patients. The evidence for platelet-rich plasma varies by condition, so we discuss whether it fits your diagnosis before recommending it. PRP is usually not covered by insurance.
Can I bring my MRI from another hospital?+
Yes. Bring the images on a disc or have them sent before your visit, along with the radiology report. We look at the images themselves, not just the report.
Will I see Dr. Morton?+
Dr. Paul Morton is the practice’s orthopedic surgeon and handles surgical decisions and operations. Depending on the reason for your visit, you may also see physician assistant Justin Grandalen, one of our physical therapists, or other members of the orthopedic team.
