For Referring Providers
Refer a patient to Pacific Bone & Joint
Thank you for trusting us with your patient. Whether you are a primary care physician, an emergency department, or a physical therapist, referring is simple: fax us the essentials or call, and we take it from there.
Dr. Paul Norio Morton, MD, and Justin Kapono Grandalen, PA, see patients at four clinics on Oahu and the Big Island. One phone number and one fax reach all of them.

How to Refer
Fax or call — please include
- Patient demographics and the best phone number to reach them
- Insurance information, plus the referral or authorization if the plan requires one
- Your clinic notes and the reason for referral
- Imaging reports — and the images themselves, uploaded through our imaging link or sent on disc with the patient
- Any urgency: new fracture, post-operative concern, or time-sensitive decision
Once we receive the referral, our team verifies the patient's benefits and calls the patient to schedule. Patients may also request an appointment themselves on Klara.
Urgent new fractures
Call the main line
For a new fracture or other time-sensitive injury, please call (808) 439-6201 rather than faxing, so we can arrange prompt follow-up. After clinic hours the line is answered by our answering service, which reaches the on-call provider.
What you receive back
Consult notes by fax
Your office receives our consultation notes by fax after the visit, so you know what we found and what the plan is. Please make sure the referring provider's fax number is on the referral.
What We Treat
Conditions and services we accept referrals for
Big Island patients
Hilo and Kona clinics, surgery in Hilo
Hawai‘i Island patients are seen in Hilo (75 Pu‘uhonu Place, Suite 204) and Kona (75-184 Hualalai Road, Suite 205, by appointment only), with surgery performed at Hilo Community Surgery Center. Physical therapy on the Big Island is coordinated with local therapy partners; our in-house PT team works on Oahu only.
Second opinions
Complex cases and surgical second opinions
We welcome referrals for second opinions on painful or loose joint replacements, nonunions, and surgical decisions. Operative reports, implant information, and uploaded imaging make these visits far more productive. Video visits are available for patients who have imaging in hand.

Referral questions, answered
How do I send a referral?+
Fax it to (808) 439-6202 with the patient’s demographics and phone number, insurance information (and the referral or authorization if the plan requires one), your notes, and imaging reports. Images can be uploaded through our secure imaging link. For urgent new fractures, call (808) 439-6201 instead.
Which plans need an authorization before the visit?+
Ohana Medicare, AlohaCare, QUEST (Medicaid), VA, and Tricare Prime require a referral or prior authorization before we can see the patient. Our Insurance page lists the plans we accept.
Do you accept workers’ compensation and auto accident referrals?+
Yes, at all four clinics. Please include the employer and carrier, claim number, and adjuster contact for work injuries, or the PIP carrier and claim number for auto accidents. See our Workers’ Compensation page for details.
Can you see my patient by video?+
Yes. Everyone is offered video visits, which work well for imaging review, second opinions, and neighbor-island consultations when imaging has been uploaded ahead of time. Anything that needs a hands-on exam, new imaging, or a procedure is scheduled in person.
Do you perform shoulder surgery?+
No. Our shoulder and elbow care is non-surgical. When surgery is genuinely needed, we coordinate a referral to a trusted shoulder specialist.
