Ultrasound-Guided Procedures
Musculoskeletal ultrasound lets your orthopedic provider see tendons, joints, nerves, and fluid in real time, right in the exam room. It helps confirm a diagnosis during the visit, and it lets a needle or small instrument be guided to exactly the right spot instead of relying on surface landmarks alone.
Pacific Bone & Joint uses ultrasound for diagnosis, for injections into joints, tendons, and bursae, for placing PRP and BMAC, and for minimally invasive procedures such as carpal tunnel and trigger finger release that can often be done in the office with local anesthesia.

Most guided injections take only a few minutes, and many guided procedures are done in the office rather than an operating room.

Diagnostic scan
The provider scans the painful area to confirm the diagnosis and plan the safest path, identifying nerves and blood vessels to avoid.

Preparation
The skin is cleaned, and local anesthetic numbs the area. For procedures such as carpal tunnel release, the hand or wrist is prepared like any sterile procedure.

Guided treatment
The needle or instrument is watched on the screen in real time as it reaches the target.

Recovery and follow-up
Most patients go home right away. Your plan may include physical therapy, a brace, or MLS laser therapy, with follow-up to check the result.

Physical therapy for recovery after guided procedures
PB&J Physical Therapy works inside our Honolulu and Waipahu/Kunia clinics, so your rehabilitation is planned with the same orthopedic team that treats you. Many orthopedic problems improve with therapy alone, and Hawai‘i allows direct access to physical therapy without a physician referral (some plans still require one for coverage).

Why use ultrasound for an injection?+
Ultrasound shows the needle reaching its target in real time. Research shows guided injections are more accurate than landmark-guided injections for many joints and tendons, which matters most for deep or small targets such as the hip joint.
Does an ultrasound-guided injection hurt?+
Most patients feel a brief pinch and pressure. The skin is numbed first, and the procedure usually takes only a few minutes.
How is ultrasound-guided carpal tunnel release different from open surgery?+
The ligament is released through a very small incision while the surgeon watches the nerve and surrounding structures on ultrasound. It is usually done in the office with local anesthesia, and many patients return to light hand use quickly. Candidacy depends on your exam and testing.
Is it covered by insurance?+
Many guided injections and procedures are covered when medically necessary; coverage depends on your plan. PRP and BMAC are usually not covered by insurance. Our team will explain any cost before treatment.
Where are ultrasound-guided procedures available?+
Diagnostic ultrasound and guided injections are available at our O‘ahu clinics, and ultrasound-guided carpal tunnel release is offered at all four clinics: Honolulu, Waipahu/Kunia, Hilo, and Kona.
Do I need imaging before the procedure?+
The ultrasound itself is part of the visit. X-rays or an MRI may still be useful to understand the whole picture. If you have outside imaging, upload it before your appointment.
