Orthopedic Care for North Shore

Orthopedic Care for Patients in North Shore, Hawai‘i

The North Shore, from Waialua and Hale‘iwa to Pūpūkea and Sunset Beach, has some of the most demanding surf in the world. It is also home to farm workers, small-business owners, and families who often drive a long way for specialty care. Surfing injuries, falls on reef and rock, and years of physical work all lead patients to an orthopedic clinic.

North Shore patients are seen at our Waipahu/Kunia clinic in the Kunia Shopping Center, usually 35 to 50 minutes away by Kamehameha Highway and H-2, or by Kunia Road. Because the trip takes time, we try to make each visit count, with imaging, evaluation, and a clear plan on the first day.

Complete Orthopedic Care

Orthopedic care in one coordinated system

From diagnosis and physical therapy to injections, advanced procedures, surgery, and recovery. Not every orthopedic problem needs surgery, and Pacific Bone & Joint is set up to move you through the right level of care instead of pushing everyone toward one treatment.

  1. 1EvaluationA focused history and exam to find the source of the problem, not just the location of the pain.
  2. 2ImagingX-ray and diagnostic ultrasound are available in the office, so most problems can be imaged during the visit. MRI is arranged when it will change the plan.
  3. 3Physical therapyPB&J Physical Therapy works inside our Honolulu and Waipahu/Kunia clinics.
  4. 4InjectionsCorticosteroid and other injections, with ultrasound guidance when accuracy matters, and PRP or BMAC for selected patients.
  5. 5Office proceduresUltrasound-guided carpal tunnel release and trigger finger release, with local anesthesia and no hospital stay for most patients.
  6. 6SurgeryArthroscopy, ACL and meniscus surgery, cartilage restoration, fracture care, and hip and knee replacement, including revision.
  7. 7RecoveryPost-operative protocols, bracing, and follow-up coordinated by the same team that planned your care.

Who will I see?

Pacific Bone & Joint is a team-based orthopedic practice. Depending on the reason for your visit, your care may involve an orthopedic surgeon, a physician assistant, a physical therapist, imaging staff, or another member of the orthopedic team. Surgical decisions and operations are handled by the surgeon. Other parts of your evaluation, rehabilitation, follow-up, injections, and recovery may involve other clinicians.

Common Conditions We Treat

What brings North Shore patients to us

Surfing shoulder injuries

Dislocations, rotator cuff strains, and AC joint separations from wipeouts and board impacts.

Knee injuries

ACL and meniscus tears from twisting in the surf, on skateboards, and on trails.

Ankle and foot injuries

Sprains and fractures from reef, rocks, and uneven ground.

Wrist and hand fractures

Falls on an outstretched hand. Some wrist fractures feel like a sprain at first.

Arthritis from physical work

Hips, knees, and hands worn down by years of farm, trade, and ocean work.

Tendon pain

Elbow, wrist, and Achilles tendon problems from paddling, farm work, and running.

Physical Therapy and Rehabilitation

Orthopedics and physical therapy, coordinated

Many orthopedic conditions improve without surgery. PB&J Physical Therapy works inside our Honolulu and Waipahu/Kunia clinics, so you can move from diagnosis to rehabilitation with one coordinated plan, and your therapist and orthopedic provider can talk about your progress.

You do not need to be a surgical patient to do therapy with us, and Hawai‘i allows direct access to physical therapy without a physician referral (some plans still require one for coverage). Not every patient needs therapy; it is recommended when it fits the problem.

Nonsurgical Options

Start with the least invasive reasonable option

Most patients start here. Many never need anything more.

Surgical Options When Needed

Advanced surgery within the same practice

When surgery is needed, it is performed at an O‘ahu surgical facility chosen for the procedure. Planning, follow-up, and physical therapy can stay at the Waipahu/Kunia clinic, the closest to home.

Why patients travel to Pacific Bone & Joint

The reason is the whole system, not one treatment

For many patients, the reason to make the trip is not one specific injection or procedure. It is having the problem evaluated by a practice that can offer physical therapy, bracing, injections, ultrasound-guided care, cartilage restoration, sports surgery, joint replacement, revision surgery, and rehabilitation when each is appropriate. The plan fits the problem, and no step depends on finding a new organization.

If you are coming from North Shore, sending imaging ahead and booking an early appointment usually means the first visit ends with a clear diagnosis and plan.

Patient Reviews

What patients say

4.9 average across 504 ratings on 8 review sites

“Dr. Morton is the best. He did my hip replacement and carpal tunnel surgeries. I would not use anyone else.”

— Stephen P.· Google review

“Dr. Paul Morton performed my anterior hip replacement, and the entire experience exceeded my expectations. I went home the same day, was off my walker within a day and a half, off my cane after just five days, and was walking up and down stairs normally 10 days after surgery. I know everyone’s recovery is different. The pain that had been limiting my life is gone.”

Common Questions

Questions from North Shore patients

Can I be evaluated and receive PRP on the same day?+

Sometimes. If you are driving in from the North Shore, an early appointment and sending your imaging ahead of time may make same-day evaluation and PRP treatment possible when it is clinically appropriate. Treatment is never guaranteed before the evaluation, because the first step is deciding whether PRP is actually the right treatment.

I just got hurt surfing. Where should I go?+

For a bone through the skin, a badly deformed limb, numbness, a head or neck injury, or a shoulder that is still out of place, go to the nearest emergency department. For sprains, suspected fractures, and shoulder or knee injuries that are not emergencies, Wiki Wiki Orthopedics at the Waipahu/Kunia clinic accepts walk-ins.

Do you help surfers get back in the water?+

Yes. Rehabilitation after a shoulder, knee, or ankle injury follows a criteria-based return-to-sport plan, so you go back when strength and stability are ready, not just when a set number of weeks has passed.

Can physical therapy be done closer to home?+

PB&J Physical Therapy works at our Waipahu/Kunia and Honolulu clinics. If a therapist closer to the North Shore makes more sense for you, we can send a referral and your treatment plan to the therapist you choose.

Do I need an MRI before my first appointment?+

No. X-ray and diagnostic ultrasound are available in the office, and an MRI is ordered only if it will change the plan. If you already have an MRI, bring the images on a disc or have them sent ahead.
