Neighbor-island patients from Maui, Kaua‘i, Moloka‘i, and Lāna‘i fly to O‘ahu to see us. Some want a second opinion. Others need a treatment that is hard to find at home, such as cartilage restoration, ultrasound-guided procedures, or complex joint replacement and revision surgery. Some want orthobiologic treatment such as PRP as part of a plan that also includes rehabilitation.

Most neighbor-island patients come to our Honolulu clinic in the Ala Moana Medical Building, a short drive from Daniel K. Inouye International Airport. Planning matters. If you send your imaging and records before you travel and book an early appointment, the first visit can usually include a full evaluation and a clear plan, and sometimes treatment on the same day.

Who will I see?

Pacific Bone & Joint is a team-based orthopedic practice. Depending on the reason for your visit, your care may involve an orthopedic surgeon, a physician assistant, a physical therapist, imaging staff, or another member of the orthopedic team. Surgical decisions and operations are handled by the surgeon. Other parts of your evaluation, rehabilitation, follow-up, injections, and recovery may involve other clinicians.

Many orthopedic conditions improve without surgery. When physical therapy is part of your plan, you can do it on your home island: we send your treatment plan or post-operative protocol to a therapist near you. While you are on O‘ahu, you can also see PB&J Physical Therapy, our in-house team at the Honolulu clinic.

Hawai‘i allows direct access to physical therapy, although some insurance plans still require a referral for coverage.

Surgical Options When Needed

Advanced surgery within the same practice

Surgery for neighbor-island patients is performed on O‘ahu, at The Queen’s Medical Center or Adventist Health Castle (robotic knee), or at another O‘ahu facility chosen for the procedure. Robotic hip replacement is performed at Hilo Community Surgery Center on Hawai‘i Island. Your team will tell you how long to plan to stay before flying home.

Getting to Pacific Bone & Joint

Where Maui, Kaua‘i, Moloka‘i & Lāna‘i patients are seen

Flights from Kahului and Līhu‘e to Honolulu run throughout the day, and Moloka‘i and Lāna‘i are served by smaller commuter flights. From the airport, the Honolulu clinic is about 15 to 25 minutes by car, taxi, or rideshare.

To make the most of one trip, book a morning appointment, send imaging and records ahead, and plan a later return flight in case you need additional imaging or a procedure the same day. Flight times are approximate.

Can I be evaluated and receive PRP on the same day?+

Sometimes. If you are flying in from another island, an early appointment and sending your imaging ahead of time may make same-day evaluation and PRP treatment possible when it is clinically appropriate. Treatment is never guaranteed before the evaluation, because the first step is deciding whether PRP is actually the right treatment.

How should I plan my trip?+

Book a morning appointment, send your X-rays or MRI on disc with the reports before you come, bring a list of medications and any operative reports, and book a return flight later in the day so you are not rushed.

Can I get a second opinion?+

Yes. Bring or send your imaging, reports, and records from your current doctor. A second opinion may confirm the plan you already have, offer other options, or explain why a treatment is or is not a good fit.

How long do I need to stay on O‘ahu after surgery?+

It depends on the procedure and your recovery. Your care team will tell you how long to plan to stay near the surgical facility and when it is reasonable to fly home.

Can my physical therapy be done on my home island?+

Yes. After your evaluation or surgery, we can send your treatment plan or post-operative protocol to a physical therapist near home. While you are on O‘ahu, you can also see PB&J Physical Therapy at the Honolulu clinic.

Will my insurance cover care on O‘ahu?+

We accept most Hawai‘i insurance plans, and some plans require a referral or prior authorization first. PRP and BMAC are usually not covered by insurance. Call (808) 439-6201 to check before you book travel.

Will I see Dr. Morton?+

Dr. Paul Morton is the practice’s orthopedic surgeon and handles surgical decisions and operations. Depending on the reason for your visit, you may also see physician assistant Justin Grandalen, a physical therapist, or other members of the orthopedic team.
